1. What are the eligible ages of member participation?

Answer: We accept members ages Pre-K to (18) years of age for the Sick-at-School/Daycare Pick-Up.   


2. What types of illnesses are available for scheduled Sick-At-School/Daycare pick-up?

Answer: We pick-up any child that has a non-emergency illnesses.  If a child is experiencing any life threatening medical conditions we will not pick that child up.  If a child's condition worsens while en-route, that child will be taken to the nearest hospital emergency room and the parent/guardian will be notified immediately to meet Zakai Care Transportation staff.  


3. What are the fee  schedules for Sick-At-School/Daycare pick-up services?

Answer: All Memberships covers all children in the household which is $4.99 monthly.  There is an additional fee of $15.00 for each scheduled Sick-At-School pick-up. Member's credit card will be billed at completion of each round-trip pick-up/drop-off service along with scheduled monthly fees.


4. What security protocols are in place for children when they are dropped off and there is no parent/guardian at home?

Answer:  We will not drop off a child when their condition worsens without someone over the age of (16) years of age, regardless of age.  We will drop off a child at home alone over the age of (12) with consent form from a parent/guardian and their condition hasn't worsened or is life threatening.


5. What age can a child be dropped off at home alone without a parent/guardian?

Answer: Prior to authorizing Membership Services for minor children, the parent/guardian must complete sign and submit forms to: Zakai Care Transportation Services Consent Form to:


Authorize the use of transport; and

Confirm the parent or guardian understands the transportation rules and the procedures to be followed by the driver if a responsible adult is not home at the time of the drop off.

The consent form provides multiple options for listing alternative drop off names, addresses and phone #’s for those times that the parent/guardian cannot be at home for the drop off.  All individuals listed on the consent must be (18) or older.  The expectation is that all are made aware they are listed on the consent and agree to serve as an alternative drop off. 

The alternative drop-off must be listed at the time of application, and there must be a party available to complete a legal transfer of custody of the age of (18).  Ongoing planned alternative drop off names and locations are provided through the initial application membership for services. 

If no responsible party is available at the time of the child’s drop-off, the transportation provider will attempt to reach the following parties in the order listed below : 

 

•       Parent/guardian by phone

•       All alternate phone numbers listed for the parent/guardian or other caregivers authorized to receive the child in lieu of the parent/guardian

•       Emergency contact for 241-KIDS and the Zakai Care Director of Safety (within one hour as a critical incident)

•       Zakai Care will advise the parent/guardian of the next steps taken and the drop off location of the child.  Zakai Care – Transportation Incident Report will be submitted within 24 hours to Director of Safety with Zakai Care.Zakai Care recommends not to  leave a child under the age of (18) years old home alone.  Zakai Care will not leave a child where there's a life threatening condition regardless of age. Zakai Care Services are offered for ages (6)years of age -(18)years of age.


6. What type of background checks are in place for Zakai Care's personnel?

Answer: We have a dash camera video capabilities on our vehicles, to protect your children and our staff.

  1. State and federal criminal history check using the individual's name.

  2. State and federal criminal history check using the individual's fingerprints

  3. Child abuse registry check (Child Protection Index)

  4. Sex offender registry check


7. How would I know as a parent/guardian that my child has arrived safely at home  or destination and secure.

Answer: During your child's transit our staff can track the ride in real-time to alert the parent that the child/teen has reached their destination via email, text, or phone.
